Visits Include

If your medical condition requires it (ie. hypertensive, hyperlipidemic, diabetic, or medications requiring blood tests or blood pressure monitoring), you will need to come for office visits every four months

Referral of patient for any Concierge Service Plan saves you 10% off your one year plan

One year contract cancellable upon 30-day notice

One year contract may be paid semi-annually or yearly

Cancellation of contract at any time for a valid reason is at Dr. Martinho’s discretion
